Care Fees Enquiry Form – initial enquiry

If your relative needs full-time care for health needs, the NHS may have a duty to pay 100% of the care fees – including all the costs of being in a care home.

You can also claim back care fees retrospectively, including if your relative has passed away but was paying care fees before they died.

What relationship to you is/was the person needing care (e.g. mother, father, aunt, etc)?

Does your relative have health needs? Please provide just brief details, e.g. Parkinson's, stroke, dementia, etc

Is your relative able to make decisions about their own care?

If not, do you currently have a Lasting Power of Attorney?

Where is he/she currently based - at home, in hospital or in a care home?

If in hospital, is the hospital putting pressure on you to move your relative elsewhere?

If in hospital, has a Section 2 notice been issued (if you know)?

Is your relative already receiving full-time care - either at home or in a care home?

In which country of the UK does he/she live (England, Wales, Scotland or N.Ireland)?

Does he/she have savings, property or other assets above the current local authority means-test thresholds? (There's no need to tell us what those assets are.)

Has he/she had a local authority financial assessment?

Is he/she already paying care fees?

Has he/she had an assessment for NHS Continuing Healthcare?

If yes, what assessment form was used (i.e. Checklist or Decision Support Tool - if you know) and what was the outcome?

Is he/she receiving the weekly Registered Nursing Care Contribution?
